SimpliGov - IT Loaner Equipment Request
Description
Build a SimpliGov Form for requesting Loaner Equipment.
Problem Statement
DOIT manages some IT equipment on a loaner basis and currently accepts requests to reserve it via email. Email does not ensure all the necessary info for a reservation.
Project Justification
This will simplify and streamline requests for IT loaner equipment. It was selected as one of 4 initial workflows that are entirely within DOIT as pilot SimpliGov workflows.

asap as pilot example to launch DEP use of SimpliGov, balanced by lack of full-time staffing for SimpliGov Center of Excellence. Targeting 6-week window from start, delayed while awaiting access to Active Directory via NJOIT.
