Contractor and BCPA LSLR (Lead Service Line Replacement) Submission Forms

Description

Based on the data gaps identified through the Newark LSLR Compliance investigation in 2024, the Water Resource Management Program is working to revise the data retention and submission requirements for contractors performing lead service line replacements in NJ. These enhanced requirements are aimed at preventing incomplete or inconsistent replacements from happening again. One of the components of the proposed enhancements is a Survey123 GIS form to be shared with contractors actively working on replacement programs, enabling them to submit documentation directly to the Department; and another is a Survey123 GIS form for the DEP Bureau of Construction, Payments, and Administration (BCPA) to fill out when performing drop-in inspections of active projects.

Project Justification

High profile issue made worse by contractor fraud. Items below also an issue.
(1) The utility owner did not own a copy or otherwise control the database in which the site inspection data collected by the engineering oversight manager was kept.
(2) Address-level site inspection reports, generated using the eBuilder database, included inconsistent levels of detail and stored key information in difficult-to-query unstructured text fields.
(3) There was no recordkeeping of the exact crew and contractor/sub-contractor that performed the LSL replacement work.
(4) Pictures taken of the work performed were inconsistent, and many pictures were submitted for the wrong address.
(5) Payment applications did not contain searchable spreadsheet files containing an address-level inventory of the work performed for which payment was sought.

I made Angela Witcher aware of this project via a Project IT Sheet on 3/19/25. I’m almost certain this is a BGIS project since the mobile app we be modeled after an existing one that was recently created.
