Need to Create an Online submission Work Flow for Ethics Personal and Business Relationships Disclosure (PBRD) Form
Description
On Friday 1/10/25, DOIT had a new production deployment for the Personal Business Relationship Disclosure Form. This deployment is an initial deployment to help streamline the process of submitting these forms. This change will save time on unnecessary emails seeking more information between the user and Ethics Office for these submissions. It was done using the new SimpliGov workflow and form development system.
Problem Statement
Existing Ethics Personal and Business Relationships Disclosure (PBRD) Form is a fillable PDF, need it to be an online automated submission work flow.
Project Justification
Automated work flow will increase efficiency and ease of submission/review of this form
